Medical service fees are collected at the time of service.
We accept VISA, MasterCard, American Express, Discover Card, personal checks or cash.

Insurance

SunDance™ accepts all major insurances for mental health. These services include but are not limited to: Blue Cross Blue Shield, Aetna, IHC/SelectHealth, PEHP, Humana, Medicaid, Medicare, Cigna, WISE Network, United Health Care, DMBA, EMI, Magellan, Humana, and U of U Health Plans.

Most insurance companies require a copayment and/or a deductible to be paid by the client at the time of service. Many require authorization prior to seeing a provider or doctor.  To avoid surprises, we suggest contacting your medical insurance provider to be sure you understand the benefit and required procedures.

Client Pay

SunDance™ offers fee-for-service rates for clients choosing not to use insurance.
